As a 4ward North PhD Stage 2 Applicant we would like you to complete this anonymous equality, diversity and inclusion monitoring survey which will take approximately 5 minutes to complete and will be available up to 30th May 2022.

Why is the survey being done?

4ward North is committed to ensure diverse and inclusive participation. The purpose of this survey is to monitor applications to the Clinical PhD Fellowship programme, which is required by Wellcome, and for us to use the data to improve the equality, diversity and inclusion of the programme. We would encourage all applicants to respond and have included ‘prefer not to say’ options if you do not want to answer specific questions. The questions reflect Wellcome's Diversity and Inclusion Survey (DAISY) guidance ( Data will be collected using the JISC Online Survey; data is secure, strict information security standards are followed (ISO27001) and data is handled in compliance with GDPR. Individual data will not be published or shared with any organisation; collated (unidentifiable) data will inform our programme guidance and eligibility. 

Do I have to take part?

No, it is up to you to decide whether or not you want to take part. Before participating, you will be asked to read a consent page and give implicit consent to take part by clicking on the ‘Next → ’ button at the bottom of the consent page. If you start the survey but change your mind, you can withdraw at any time and without giving a reason by closing the browser window and your responses will not be recorded.

What will happen if I decide to take part?

If you decide to take part in this anonymous online survey after reading this privacy notice the survey can be accessed after you complete the following consent page. The survey will involve responding to a brief set of questions, there is no right or wrong way of responding to the questions; simply select the response which is most true for you.

What are the possible risks of taking part?

There are no known risks of taking part in this survey.  You do not need to answer any questions that you do not want to.  All data collected is anonymous.

What are the possible benefits of taking part?

The information we collect will help us monitor applications to our 4ward North clinical PhD programme.

Will my taking part in this project be kept confidential?

All data collected will be anonymous, as we do not ask for any personally identifying information e.g. name, address, phone number. Your responses to this survey cannot be linked to your application. Data will be stored securely in line with ethical and legal practices and no identifiable data will be published. All of the safeguards offered by this survey software to protect your identity (including your IP address) have been activated.  Your anonymity will be preserved at all times.
